tier 1

This plan is for patients who need moderate to significant health changes. You've tried other approaches, and nothing has stuck long-term. If you've struggled with bad habits and behaviors for years, even decades, and want to un-do patterns to create a new relationship with yourself, this program is for you.

Tier One Includes:
  • Initial 90-minute client assessment and onboarding session (in-person or virtual)
  • Weekly 45-minute visits (in-person or virtual)
  • Weekly check-in between visits (via Power2Patient)

$1,725 for 3-month program: $575/month

tier 2

This plan is for patients who need minimal to moderate health changes. You have some foundation of positive habits, self-accountability, and awareness of what works for them and what doesn't. You want expert support to take your baseline to the next level.

Tier Two Includes:
  • Initial 90-minute client assessment and onboarding session (in-person or virtual)
  • Bi-weekly 45-minute visits (in-person or virtual)
  • Weekly check-in between visits (via Power2Patient)

$1,050 for 3-month program; $350/month

Learn. Gain knowledge about foundational health habits and how to integrate them into your life.

Intentional. Cut through the noise of what influencers and fake “gurus” tell you you’re “supposed” to be doing and set an intentional wellness vision + supporting practices that are meaningful to you.

What is Fountain's health coaching style?

At this time, health coaching is unregulated. There's a move to change this to ensure all coaches have the appropriate training and credentials.
